Transitional Homes for Medically Fragile Children

Specialized care for children with complex medical needs

KenCrest operates two transitional care homes in Philadelphia, Pennsylvania that support medically fragile children and/or technology-dependent children with various diagnoses, including tracheostomies, central lines, oxygen and feeding tubes, chromosomal disorders, and neurological conditions.

KenCrest was one of the first organizations in the country to pioneer this specialized care model empowering families to move their young children from the hospital to a more home-like setting. Each KenCrest home offers a warm, family-centered setting where children thrive in a community-based life model by participating in field trips, attending school programs, spending afternoons at the park, and other daily activities while receiving the specialized rehabilitative services they need.

Two nurses and one direct support professional provide exceptional care 24 hours daily.
