KenCrest Transitional Care Homes for Children

Transitional Care Homes for Children with Medical NeedsKenCrest’s transitional care homes are specifically designed to care for infants, toddlers and children with complex medical needs. They are licensed by the PA Department of Health as Intermediate Care homes for infants and young children who are medically fragile and/or technology-dependent.

Our homes are uniquely set up to receive children of all ages who are medically ready to be discharged from the hospital but require some type of ongoing support beyond that which they can receive at their family home.

These homes offer a warm, inviting environment that provides for their medical needs and encourages and supports natural family involvement.

Each child is fully supported to continue healing, and grow both physically and emotionally in one of several of KenCrest’s transitional care homes for children until they are discharged.

Additionally, each family’s needs, specific to their child’s special care requirements, are identified. Ongoing training for giving this care is then provided to ensure a safe transition home.
