Home and Community Based Support
KenCrest provides home and community based support to adults and their families when the individual is living in a family residence. Services are currently available in Bucks, Montgomery, and Philadelphia Counties.
KenCrest’s home and community based support team, in conjunction with the individual, their family, and their service coordinator, utilizes a person-centered planning approach in creating and delivering the outcome-based supports outlined by the individual service plan (ISP). The person-centered approach takes into account the individual’s strengths, capabilities, preferences, lifestyle and cultural background, and goals to form a life-course trajectory and plan for the future.
All supports are designed to focus on each person’s growth, personal fulfillment, and individual choices and desires, encouraging an everyday life.
Varied Supports, Varied Degrees
The individuals in our home and community based supports program have various needs, making a “one size fits all” approach insufficient. Services vary greatly and are as creative as they are varied. Supports include, but are not limited to:
- Learning, maintaining, or enhancing a particular skill, such as cooking or managing finances
- Being more active in the community and joining new social groups
- Learning how to use public transportation
- Cultivation of personal care skills
- Completion of household chores or other daily living skill development
Ongoing and Specific
The supports KenCrest provides are based on individual, outcome-based goals outlined by each person’s ISP. Regular team meetings serve to discuss the progress made on each goal and adjust supports according to a person’s goals, outcomes, and interests.
The social activity club is an informal social program designed exclusively for individuals in the home and community based support program and grows by the day. The activity club participants engage in a host of community based social activities such as bowling, trips, breakfasts, and many others.
