Transitional Care Homes for Children with Complex Medical Needs

If your child has complex medical needs, we can help. KenCrest has two transitional care homes for infants and young children who are ready to be discharged from the hospital, but whose families cannot provide the required care for their child.. The goals are to:

• Assist in the growth of the child both physically and emotionally
• Enhance and strengthen bonds between parent and child
• Increase parental skills working toward reunification

These transitional care homes are located in home settings allowing the child to make an easy transition from our home back to their family’s home. The home environment greatly enhances the development, social, and medical rehabilitation of each child we service.
