Community Residential Services
Community Residential Supports at KenCrest reflect the values and desires of the individuals that live in the specific home. Supports are provided in a wide variety of homes and communities located in Bucks, Chester, Montgomery and Philadelphia counties. People supported in Community Residential homes range from school age children to older adults.
Services and support may be provided in a traditional home with up to 4 individuals or in smaller apartment settings. The staff support in each home is designed to meet the supervision needs of the individual living in the home. Most homes have 24 hour day supervision by KenCrest staff and all homes meet licensing standards established by the state of Pennsylvania.
A residential home can support from 1 to 4 persons based on:
• Need for supervision
• Personal choice
• Need for specialized care (Behavioral support)
• Preference in housemates
Community Residential Supports offer:
• Personal Needs Assessments which include looking at individual interests, relationships, dreams, skills and life circumstances.
• Opportunities to develop personal relationships and participate in community.
• Direct Support Staff trained in the individual needs, personality and personal desires of each individual.
• Medical care coordination.
• Support in obtaining ancillary clinical support (link to Clinical Support)or evaluations in communication, Physical or Occupational therapy, sexuality, mobility, nutrition, or vision.
• Up to 24 hour /7 days a week direct staff support.
• 24 hour / 7 days a week emergency response.
• 24 hour / 7 days a week nursing support.
• Assistance in obtaining and managing health and financial benefits.
KenCrest also provides Support Living assistance. This service provides support to individuals who live by themselves or with a companion and are able to choose targeted areas for assistance. This may include assistance with shopping, finances, obtaining benefits or medical coordination.
Availability:
Services are currently available in Bucks, Chester, Montgomery and Philadelphia counties.
